Form 1095-C

Beginning with the 2015 tax year, employers

with 50 or more full-time equivalent employees

were required to provide health insurance, furnish Form 1095-C to their employees, and file the employer copies to the IRS. If you have not filed 2015 - 2017, and/or need help with 2018,
